The same population, phenomenon and context should remain visible across all five elements.

Useful starting points include inconsistent care, conflicting evidence, an implementation gap, under-represented populations, a difficult transition, an education problem or an uncertain patient experience.
Personal observation can prompt a research idea, but it does not by itself prove that the wider problem exists. Test the concern against current literature, guidance, policy or authorised local evidence.
