A nursing research question should make the intended evidence clear before you choose a methodology or database search. PICO is useful for intervention-effectiveness questions, but it is not the right structure for every nursing enquiry. Experience, exposure, implementation and evidence-mapping questions require different logic.

This guide compares PICO, PIO, PEO, PICo, PCC and SPIDER and shows how to move from a broad nursing problem to a focused, searchable and methodologically aligned question.

Choose the framework from the question type

  • PICO: comparative intervention effectiveness.
  • PIO: intervention and outcome where a comparator is unnecessary.
  • PEO: exposure and outcome.
  • PICo: experience or perception in a defined context.
  • PCC: evidence mapping for a scoping review.
  • SPIDER: qualitative or mixed-method evidence searching.

Start with the nursing problem in plain language

Do not begin by trying to fill an acronym. Write two or three sentences explaining what is happening, who is affected and what remains uncertain.

For example:

Adults discharged after heart-failure admission must manage medicines, symptoms and follow-up at home. It remains uncertain whether structured nurse-led discharge education improves outcomes compared with routine information.

This problem naturally points toward an intervention-effectiveness question.

By contrast:

Family carers of people with dementia often participate in hospital discharge planning, but their experience of communication and coordination may not be captured by readmission statistics.

This problem asks about experience and context, so a qualitative framework such as PICo is more appropriate.

Decide what kind of answer you need

The wording should match the intended inference. A cross-sectional project should not ask a causal question it cannot answer, and a qualitative study should not be framed as though it will estimate prevalence.

Define the population carefully

State who the question concerns without adding arbitrary restrictions. “Healthcare workers” may be too broad; “newly qualified registered nurses working in adult acute-care settings” may provide a more coherent population if career stage and setting are important to the problem.

Include age, diagnosis, professional group, care setting or other characteristics only when they affect relevance or eligibility.

Define the central concept precisely

The central element changes according to the framework:

  • Intervention: an action intended to create change, such as nurse-led education.
  • Exposure: a characteristic or condition not assigned by the researcher, such as night-shift work.
  • Phenomenon of interest: an experience or process, such as shared decision-making.
  • Concept: a broader issue being mapped, such as reasonable adjustments.

Replace vague terms such as “nursing care” with the specific process or experience the dissertation will investigate.

PICO for comparative intervention questions

PICO stands for Population, Intervention, Comparator and Outcome. Cochrane uses PICO as a central structure for defining intervention-review questions and eligibility (Thomas et al., 2024).

Question: In adults with type 2 diabetes managed in primary care, how effective is structured nurse-led self-management education, compared with usual education, in improving HbA1c?

Do not invent a comparator merely to complete the acronym. If comparison is not meaningful, PIO may be cleaner.

PIO when no meaningful comparator is required

PIO retains Population, Intervention and Outcome.

Example: How do nurse-led distraction techniques affect pain and anxiety among hospitalised children undergoing venepuncture?

Removing the comparator changes what the question can establish. Use PIO only when a comparison is genuinely unnecessary rather than inconvenient.

PEO for exposure and outcome

PEO stands for Population, Exposure and Outcome and is useful when the central factor is not assigned as an intervention.

Example: Among registered nurses working in acute hospitals, how is regular night-shift work associated with sleep quality and burnout?

The verb “associated” is appropriate because the question concerns an exposure relationship rather than a controlled intervention effect.

PICo for qualitative experience questions

PICo uses Population, Phenomenon of Interest and Context.

This question asks how people experience a process. It does not ask whether discharge planning “works.”

PCC for scoping-review questions

JBI recommends Population, Concept and Context when developing scoping-review questions and inclusion criteria (Peters et al., 2024).

PCC allows broader evidence mapping, but it still requires explicit boundaries.

SPIDER for qualitative and mixed-method evidence

SPIDER stands for Sample, Phenomenon of Interest, Design, Evaluation and Research type and was developed as an alternative framework for qualitative and mixed-method evidence synthesis (Cooke et al., 2012).

For example:

  • Sample: newly qualified mental health nurses;
  • Phenomenon: supporting people experiencing suicidal distress;
  • Design: interviews or focus groups;
  • Evaluation: experiences and perceived support needs;
  • Research type: qualitative or mixed methods.

The final research question can remain concise: How do newly qualified mental health nurses experience supporting people in suicidal distress in acute inpatient settings?

SPIDER may improve specificity in some qualitative searches, but greater specificity can also reduce sensitivity. Methley et al. (2014) found this trade-off when comparing SPIDER with PICO.

Do not treat the framework as the complete database search

A framework defines the question; a search strategy translates its concepts into database language. Searching every PICO element can reduce retrieval because comparators and outcomes are not always well represented in titles and abstracts.

Research on PICO searching has shown that using the full framework does not automatically improve retrieval and that comparator and outcome elements may reduce recall (Eriksen & Frandsen, 2018; Frandsen et al., 2020).

For the diabetes example, an initial search might use:

("type 2 diabetes" OR T2DM) AND ("nurse-led education" OR "self-management education")

Then test whether adding outcome or setting terms improves relevance without excluding important studies.

Test the question before approval

Apply five checks:

  1. Clarity: Can another person explain exactly what the question asks?
  2. Focus: Does it contain one main enquiry?
  3. Answerability: Does appropriate evidence exist?
  4. Method alignment: Can the proposed design answer it?
  5. Feasibility: Can it be completed within the available time, access and word count?

Then run a short scoping search. If the result set is unmanageable, narrow one concept. If almost nothing is found, check terminology and remove arbitrary restrictions before concluding that the topic lacks evidence.

A weak question improved step by step

Weak: How does nursing affect diabetes?

Better population: How does nursing affect adults with type 2 diabetes?

Better intervention/outcome: How does nurse-led self-management education affect glycaemic control among adults with type 2 diabetes?

Focused PICO: In adults with type 2 diabetes managed in primary care, how effective is structured nurse-led self-management education, compared with usual education, in improving HbA1c?

The improvement comes from clarifying the intellectual job of the study rather than adding more words for their own sake.

Common research-question mistakes

  • Writing an aim instead of a question.
  • Combining effectiveness and experience into one unmanageable enquiry.
  • Adding a comparator with no analytical purpose.
  • Forcing a qualitative question into PICO.
  • Using causal verbs for observational evidence.
  • Making the population unnecessarily narrow.
  • Treating a question framework as a fixed search formula.
  • Choosing a methodology before deciding what evidence is needed.

Research-question checklist

  • The question addresses one clear nursing problem.
  • The population is defined and defensible.
  • The intervention, exposure, phenomenon or concept is precise.
  • A comparator is included only when it matters.
  • The outcome, experience or context matches the intended design.
  • The question can be translated into search concepts or variables.
  • Enough relevant evidence appears to exist.
  • The proposed methodology can answer the question.

Frequently asked questions

Which framework is best?

There is no universal best framework. Choose the one that matches the question type and does not distort the enquiry.

Can PICO be used for qualitative research?

It can sometimes be adapted, but PICo or SPIDER often represent experience and context more naturally.

What is the difference between PIO and PEO?

PIO examines an intervention and outcome. PEO examines an exposure and its relationship with an outcome.

Does every PICO question need a comparator?

No. If no meaningful comparison exists, PIO may be more appropriate.

Can the framework change after the preliminary search?

Yes. Refinement is normal when the first search shows that the population is too narrow, the comparator is poorly defined or the available evidence differs from expectations.
